This study shows that combining ankle fusion with tibial lengthening using an internal nail effectively corrects limb length discrepancy and improves function. Patients experienced significant improvement in limb length and mobility post-surgery.
Area of Science:
- Orthopedic Surgery
- Biomedical Engineering
- Reconstructive Surgery
Background:
- Limb length discrepancy (LLD) and ankle deformity present significant challenges in orthopedic surgery.
- Traditional treatments often involve separate procedures for fusion and lengthening, potentially leading to suboptimal outcomes.
Observation:
- A novel technique combining ankle fusion with simultaneous tibial lengthening using a magnetic internal lengthening nail was employed in 17 patients.
- Preoperative limb length discrepancy averaged 4.9 cm, with concurrent ankle deformity present in all cases.
Findings:
- The combined procedure achieved a mean tibial lengthening of 4 cm, significantly reducing the final LLD to a mean of 1 cm (p<0.01).
- All patients achieved a plantigrade foot position with minimal to no pain and improved functional walking post-surgery.
- Radiographic and clinical outcomes confirmed successful ankle fusion and limb lengthening in all cases.
Implications:
- This combined approach offers an effective solution for patients requiring both ankle fusion and limb lengthening, simplifying treatment and improving patient function.
- This is the first report detailing the successful use of an internal lengthening nail for simultaneous ankle fusion and tibial lengthening.
- The technique holds promise for improving outcomes in complex lower limb deformities and discrepancies.
