Perivascular epithelioid cell tumors (PEComas) are rare mesenchymal tumors. This case highlights a PEComa detected incidentally in the cecum during a routine colonoscopy, emphasizing the need for vigilance in diagnosing these unusual gastrointestinal neoplasms.
