Light rays enter the eye through the cornea, a transparent dome-shaped tissue that is the eye's outermost layer. The cornea bends or refracts, light rays traveling to the pupil. Background:

Purpose of the Study:

  • To compare longitudinal changes in higher-order wavefront aberrations and image quality following bilateral PRK, FS-LASIK, and SMILE for myopia correction.
  • To provide an optical explanation for observed differences in visual performance post-refractive surgery.

Main Methods:

  • 106 subjects underwent bilateral PRK, FS-LASIK, or SMILE for myopia.
  • Wavefront aberrations and image quality were assessed pre-operatively and at multiple post-operative intervals (1 week to 6 months) using computational through-focus analysis.
  • Image quality metrics included peak value, interocular difference, best focus, and depth of focus for a 6-mm pupil.

Main Results:

  • SMILE resulted in a significantly smaller increase in higher-order aberrations compared to PRK and FS-LASIK.
  • PRK and FS-LASIK led to significant reductions in peak image quality, increased interocular differences, myopic shifts in best focus, and widened depth of focus.
  • SMILE showed only negligible changes in these image quality parameters.

Conclusions:

  • SMILE refractive surgery maintains better image quality and interocular consistency compared to FS-LASIK and PRK.
  • The superior visual outcomes after SMILE are attributed to a lesser increase in post-operative higher-order wavefront aberrations.
