Common variable immunodeficiency (CVID) can cause severe gastrointestinal problems. Vedolizumab treatment rapidly and effectively resolved severe enteropathy in a CVID patient who did not respond to other therapies.
Area of Science:
- Immunology
- Gastroenterology
Background:
- Common variable immunodeficiency (CVID) is characterized by hypogammaglobulinemia and increased susceptibility to infections.
- Noninfectious gastrointestinal manifestations, including severe enteropathy, are increasingly recognized in CVID patients.
- Severe enteropathy in CVID can lead to malabsorption, weight loss, and debilitating diarrhea.
Observation:
- A 63-year-old male patient with CVID presented with severe enteropathy.
- The patient's condition was refractory to conventional treatments, including empiric therapies, cytomegalovirus treatment, and corticosteroids.
- Vedolizumab, a biologic agent targeting the gut, was administered.
Findings:
- Vedolizumab treatment resulted in a dramatic and rapid clinical improvement.
- The patient experienced a durable complete response, with resolution of enteropathy symptoms.
- This indicates a significant therapeutic benefit of vedolizumab in this challenging clinical scenario.
Implications:
- Vedolizumab may represent a novel and effective therapeutic option for severe enteropathy in CVID.
- Targeting gut inflammation with vedolizumab could be a viable strategy for managing refractory gastrointestinal complications in immunodeficiency disorders.
- Further research is warranted to explore the efficacy and safety of vedolizumab in a broader CVID population with gastrointestinal involvement.
