Background:

  • Ectopic thyroid tissue, commonly found in the lingual region, is often presumed to be metastatic thyroid cancer, leading to surgical resection.
  • Distinguishing between normal ectopic thyroid tissue and metastatic foci is crucial for appropriate patient management.

Observation:

  • A rare case of normal ectopic thyroid tissue identified within the sternocleidomastoid muscle in a patient with bilateral papillary thyroid cancer.
  • The presence of bilateral papillary thyroid tumors did not automatically imply the ectopic thyroid tissue was malignant.

Findings:

  • Ectopic thyroid tissue in the sternocleidomastoid muscle is exceptionally uncommon.
  • Normal ectopic thyroid tissue can coexist with confirmed papillary thyroid cancer, challenging the assumption of malignancy.
  • Surgical intervention for ectopic thyroid tissue should be selective, particularly for patients undergoing total thyroidectomy and requiring radioactive iodine therapy.

Implications:

  • This finding necessitates a re-evaluation of the surgical approach to ectopic thyroid tissue, especially in the context of thyroid cancer.
  • Conservative management of normal ectopic thyroid tissue may be appropriate, avoiding unnecessary morbidity.
  • Further research is warranted to establish clear guidelines for managing ectopic thyroid tissue in thyroid cancer patients.
