Summary

Portable, hand-held accelerometer-based navigation (ABN) provides similar total knee arthroplasty (TKA) alignment accuracy and functional outcomes compared to computer-assisted navigation (CAN), with a shorter surgery time. Long-term clinical and functional results were comparable between the two navigation methods.

Background:

  • Computer-assisted navigation (CAN) systems are utilized in total knee arthroplasty (TKA) to improve surgical accuracy.
  • Portable, hand-held accelerometer-based navigation (ABN) presents an alternative navigation technology for TKA.
  • Limited comparative studies exist on the alignment accuracy and clinical outcomes between CAN and ABN in TKA.

Purpose of the Study:

  • To compare the operative time, blood loss, and radiological, clinical, and functional outcomes of CAN-guided versus ABN-guided bilateral TKA.
  • To evaluate the long-term efficacy and accuracy of ABN compared to CAN in TKA procedures.

Main Methods:

  • Fifty patients undergoing bilateral TKA were randomized to receive either CAN-guided or ABN-guided surgery.
  • Surgical time, blood loss, and intraoperative complications were recorded.
  • Radiological alignment, American Knee Society Scores (KSS), and Oxford Knee Scores (OKS) were assessed pre-operatively and up to 48 months post-operatively.

Main Results:

  • Mean surgical time per knee was significantly lower in the ABN group (54.5 min) compared to the CAN group (61.7 min; p < 0.01).
  • No significant differences were observed in mean blood loss, post-operative mechanical axis alignment, or frontal angles between the groups.
  • At 48-month follow-up, functional KSS (89.0 vs 88.1) and OKS (40.5 vs 39.6) were comparable between ABN and CAN groups.

Conclusions:

  • Portable ABN demonstrates comparable alignment accuracy and functional outcomes to CAN in TKA, with the advantage of reduced surgical duration.
  • Neither navigation technique showed superiority in clinical or functional outcomes at the 48-month follow-up.
  • ABN is a viable alternative to CAN for TKA, offering similar efficacy with improved operative efficiency.
