Reversible congestive heart failure associated with hypocalcemia: A case report

- A 64-year-old woman presented with acute CHF, dyspnea, and chest heaviness.
- She had a history of thyroidectomy for a malignant thyroid tumor, muscle spasms, and cataracts.
- Laboratory results revealed severe hypocalcemia (serum calcium 1.33 mmol/L, ionized calcium 0.69 mmol/L) and suppressed parathyroid hormone (< 1.0 pg/mL).
Findings:
- Echocardiography showed a severely reduced ejection fraction (28.48%).
- Cardiac function rapidly improved upon correction of hypocalcemia.
- Follow-up revealed a significant increase in ejection fraction to 48.50%.
Implications:
- This case highlights the critical importance of monitoring calcium levels in patients with hypoparathyroidism.
- Timely management of hypocalcemia is essential to prevent life-threatening complications such as CHF.
- Early diagnosis and treatment can lead to rapid and significant recovery of cardiac function.
