Acute Pyelonephritis II: Diagnostic Studies and Management

66
Introduction:For diagnosing acute pyelonephritis, a comprehensive patient history is collected to identify symptoms such as dysuria, frequent or urgent urination, flank pain, or costovertebral angle (CVA) tenderness that may suggest a kidney infection.Physical ExaminationDuring the physical examination, CVA tenderness is assessed. This involves gentle percussion over the costovertebral angle, where tenderness often indicates a kidney infection.Diagnostic TestsUrinalysis: Used to identify white...

Area of Science:

  • Ophthalmology
  • Oncology
  • Medical Case Reports

Background:

  • Ocular inflammation can be a presenting sign of systemic disease.
  • Paraneoplastic syndromes involve immune responses to cancer affecting non-cancerous tissues.
  • Small-cell lung cancer (SCLC) is an aggressive form of lung cancer.

Observation:

  • A 70-year-old male presented with gradual, bilateral visual deterioration.
  • Ophthalmological examination revealed symmetrical vitritis in both eyes without other fundus abnormalities.
  • The patient reported a history of smoking, weight loss, and was diagnosed with SCLC.

Findings:

  • The patient was diagnosed with bilateral paraneoplastic vitritis.
  • Systemic evaluation confirmed small-cell lung cancer (SCLC).
  • Ocular inflammation resolved with oral steroid treatment and cancer therapy.

Implications:

  • Ocular manifestations, such as vitritis, can serve as an early indicator of underlying malignancy.
  • Ophthalmologists should consider systemic evaluation for malignancy in patients with unexplained bilateral vitritis.
  • Prompt diagnosis and treatment of SCLC, potentially aided by ophthalmological findings, can improve patient outcomes.
