Parkinson's disease (PD) may cause ischemic stroke (IS), particularly cardioembolic and large artery subtypes. Genetic analysis found no evidence that IS causes PD or affects small-vessel stroke.

Background:

  • Parkinson's disease (PD) and ischemic stroke (IS) are significant global health concerns.
  • Understanding the etiological relationship between PD and IS is crucial for effective prevention and treatment strategies.

Purpose of the Study:

  • To investigate the potential causal relationship between Parkinson's disease and ischemic stroke using Mendelian randomization.
  • To determine if PD is a causal factor for IS and its subtypes, and to assess for reverse causality.

Main Methods:

  • Utilized Mendelian randomization (MR) methods with summary statistics from large-scale genome-wide association studies (GWAS).
  • Included 33,674 PD cases and 40,585 IS cases.
  • Employed inverse variance-weighted, weighted median, MR-Egger, robust adjusted profile score, and radial regression methods for primary and sensitivity analyses, including bidirectional analyses.

  • Sensitivity analyses supported these findings, and no strong evidence for reverse causality from IS to PD was observed.

Conclusions:

  • Parkinson's disease is a potential causal factor for cardioembolic and large artery ischemic stroke.
  • There is no significant evidence that ischemic stroke causes Parkinson's disease.
  • These findings highlight a potential etiological link between PD and specific IS subtypes, informing future research and clinical considerations.
