Physician-modified stent grafts offer effective solutions for complex aortic diseases in patients with unusual anatomy. These customized grafts provide a viable alternative to high-risk open surgery, especially in urgent or challenging cases.

Area of Science:

  • Vascular Surgery
  • Endovascular Interventions
  • Aortic Disease Management

Background:

  • Physician-modified endografts (PME) are utilized for aortic diseases when standard grafts are unsuitable.
  • They serve as alternatives to high-risk open surgery or for complex anatomical configurations.

Observation:

  • Three cases involving PME implantation are presented, showcasing initial clinical experiences.
  • Cases included infrarenal aortic aneurysm with diameter mismatch and common iliac artery aneurysm.
  • Management of a type I endoleak in a ruptured abdominal aortic aneurysm with unusual anatomy was also addressed.

Findings:

  • Technical success was achieved in all three cases of physician-modified stent graft implantation.
  • Customized grafts successfully addressed diameter discrepancies and complex anatomical challenges.

  • Specific modifications included reverse mounting of an iliac limb graft and cutting a thoracic stent graft for repositioning.
  • Implications:

    • PME implantation is an effective strategy for treating patients with unusual aortic anatomy or complex pathologies.
    • This technique expands endovascular treatment options beyond standard anatomical suitability.
    • The use of PME is recommended in high-volume aortic centers for complex endovascular aortic repairs.
