This study found altered complement factor levels in narcolepsy patients, suggesting a role for immune mechanisms in this sleep disorder. These complement levels may serve as biomarkers for clinical severity.
Area of Science:
- Immunology
- Neurology
- Sleep Medicine
Background:
- Narcolepsy is a chronic sleep disorder suspected to have autoimmune origins.
- Complement-mediated immune mechanisms in narcolepsy require further investigation.
Purpose of the Study:
- To investigate the significance of classical complement pathway activation in narcolepsy.
- To explore complement factor levels and autoantibodies in narcolepsy patients.
Main Methods:
- ELISA screening of narcolepsy patients (n=42) and controls (n=26) for C1q, C3a, C4d, and C4BP levels.
- Development of a novel ELISA for detecting antibodies to C4BP-alpha (anti-C4BPA).
- Correlation analysis between complement levels and clinical narcolepsy symptoms.
Main Results:
- Narcolepsy patients exhibited significantly higher C1q and lower C4d and C4BP levels compared to controls.
- C3a levels were similar between groups.
- Anti-C4BPA antibodies were detected in 11 narcolepsy patients.
- Complement factor levels correlated with REM sleep metrics and Epworth Sleepiness Scale scores.
Conclusions:
- Complement-mediated immune mechanisms may contribute to narcolepsy pathogenesis.
- Further research is needed to clarify the role of autoantibodies in complement alterations.
- Complement factor levels show potential as biomarkers for predicting narcolepsy clinical severity.
