Abstract

Insights

Continued HER2-targeted therapy after progression shows promise for advanced HER2-positive breast cancer. Tucatinib plus trastuzumab and capecitabine significantly improves overall survival, with other agents demonstrating notable response rates.

Area of Science:

  • Oncology
  • Medical Research
  • Clinical Trials

Background:

  • Continued human epidermal growth factor receptor 2 (HER2) suppression is supported beyond progression on HER2-directed therapy for advanced HER2-positive breast cancer.
  • Recent data from phase II and III trials evaluating HER2-directed therapy after second-line T-DM1 have become available.

Purpose of the Study:

  • To systematically review and identify phase II and III trials assessing novel HER2-targeted agents as third-line or later therapy for HER2-positive advanced breast cancer.
  • To evaluate the efficacy of these agents in improving progression-free survival (PFS) and overall survival (OS).

Main Methods:

  • Systematic literature search for phase II and III trials using keywords 'breast cancer', 'HER2', 'advanced', and ('phase II' OR 'phase III').
  • Included trials assessed novel HER2-targeted agents as third-line or beyond therapy.
  • Efficacy outcomes, including PFS and OS, were analyzed.

Main Results:

  • Eight clinical trials were identified. In phase III trials, margetuximab and neratinib combinations showed modest PFS improvements but no significant OS benefit.
  • Tucatinib combined with trastuzumab and capecitabine demonstrated significant improvements in both median PFS and OS in a phase II trial, with notable benefit in patients with brain metastases.
  • Trastuzumab-deruxtecan, zenocutuzumab, and poziotinib showed benefit in phase II trials. Trastuzumab-deruxtecan exhibited the highest overall response rate (62.0%) and median duration of response (18.2 months) in heavily pretreated patients.

Conclusions:

  • Tucatinib plus trastuzumab and capecitabine significantly prolongs overall survival in patients with advanced HER2-positive breast cancer.
  • Promising preliminary response outcomes for trastuzumab-deruxtecan suggest its potential sequencing after second-line therapy is a reasonable strategy.

Targeted Cancer Therapies

The targeted cancer therapies, also known as “molecular targeted therapies,” take advantage of the molecular and genetic differences between the cancer cells and the normal cells. It needs a thorough understanding of the cancer cells to develop drugs that can target specific molecular aspects that drive the growth, progression, and spread of cancer cells without affecting the growth and survival of other normal cells in the body.
There are several types of targeted therapies against...

Treatment Resistant Cancers02:56

Treatment Resistant Cancers

Cancer is the second leading cause of death in the United States. A cancer cell is genetically unstable and hence can mutate faster. They can also modify their microenvironment and escape immune surveillance. The difficulties in treating cancer are further compounded by the emergence of rapid resistance to anticancer drugs. The most common ways to attain resistance in cancer cells include alteration in drug transport and metabolism, modification of drug target, elevated DNA damage response, or...

Combination Therapies and Personalized Medicine02:50

Combination Therapies and Personalized Medicine

Combining two or more treatment methods increases the life span of cancer patients while reducing damage to vital organs or tissue from the overuse of a single treatment. Combination therapy also targets different cancer-inducing pathways, thus reducing the chances of developing resistance to treatment.
The combination of the drug acetazolamide and sulforaphane is a good example of combination therapy to treat cancer. The cells in the interior of a large tumor often die due to the hypoxic and...

Cancer Vaccines

Cancer treatment vaccines are a rapidly evolving field that offers a promising approach to immunotherapy. Unlike traditional vaccines that prevent diseases, cancer treatment vaccines are designed to treat existing cancers by stimulating the immune system to recognize and attack cancer cells.
Cancer vaccines come in two categories: preventive (prophylactic) and treatment (active). Preventive vaccines, such as the Human Papillomavirus (HPV) vaccine, protect against viruses that cause certain...
