A COVID-19 patient presented with unusual bleeding symptoms including bloody urine, tarry stools, and skin bruising. These severe hemorrhagic manifestations highlight potential complications of SARS-CoV-2 infection.
Area of Science:
- Internal Medicine
- Infectious Diseases
- Hematology
Background:
- Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, the cause of COVID-19, has been associated with a wide spectrum of clinical manifestations.
- While respiratory symptoms are most common, thrombotic and hemorrhagic complications have been increasingly reported.
Observation:
- A 47-year-old woman with a history of COVID-19 presented with hematuria, melena (dark tarry stools), petechiae, and extensive bruising.
- She also reported recent onset of severe headache, epistaxis, and a diffuse rash.
- Symptoms began approximately 14 days after initial SARS-CoV-2 diagnosis, with a rapid escalation in bleeding complications over the preceding 3 days.
Findings:
- The patient exhibited significant bleeding manifestations, including gross hematuria and melena, indicative of gastrointestinal and urinary tract bleeding.
- Widespread bruising and petechiae suggested a consumptive coagulopathy or vascular dysfunction.
- The constellation of symptoms points towards a severe systemic inflammatory response potentially triggered by SARS-CoV-2 infection.
Implications:
- This case underscores the potential for SARS-CoV-2 to induce severe hemorrhagic complications beyond typical respiratory illness.
- Further investigation into the mechanisms by which COVID-19 affects hemostasis and vascular integrity is warranted.
- Clinicians should maintain a high index of suspicion for bleeding disorders in patients with a history of COVID-19, especially those presenting with unusual symptoms.

Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...
