Background:

  • Hemispatial neglect, a disorder of asymmetric spatial processing, is traditionally assessed using paper-and-pencil methods.
  • Computer-based assessments are increasingly recognized for their enhanced sensitivity in detecting neglect.
  • The optimal digital assessment strategy, however, remains unclear, particularly when comparing converted traditional tasks with novel dual-task paradigms.

Purpose of the Study:

  • To compare the sensitivity of a digitally converted cancellation task versus a computer-based visual and auditory dual-tasking approach in identifying contralesional omissions in patients with chronic unilateral stroke.
  • To evaluate whether a dual-task paradigm offers superior detection of subtle visuospatial deficits compared to a digital cancellation task.

Main Methods:

  • Forty patients with chronic unilateral stroke (20 right hemisphere, 20 left hemisphere) and 20 healthy controls participated.
  • A cancellation task was administered in both large-screen and A4 paper formats.
  • Computer-based visual and auditory dual-tasks were presented on a 15-inch monitor, requiring detection of lateralized targets.

Main Results:

  • The cancellation task, in either format, failed to detect spatial bias in right hemisphere patients.
  • Right hemisphere patients demonstrated significantly more missed left-sided targets than controls in the visual dual-task.
  • In the auditory dual-task, right hemisphere patients missed more left-sided than right-sided targets, specifically during bilateral trials.

Conclusions:

  • Computer-based dual-tasking significantly outperforms digital cancellation tasks in detecting hemispatial neglect, even when the latter is presented on a large screen.
  • Attentionally demanding assessment methods are crucial for uncovering mild forms of contralesional visuospatial deficits.
  • Dual-task paradigms represent a more sensitive approach for the assessment of spatial processing disorders post-stroke.
