Background:

Ectopic pregnancy is rare, with diaphragmatic implantation being exceptionally uncommon.
This case highlights a unique instance of abdominal pregnancy located beneath the diaphragm.

Observation:

A 30-year-old woman presented with intermittent abdominal pain and a positive pregnancy test.
Imaging revealed a mass under the diaphragm with features suggestive of ectopic pregnancy, including a yolk sac.

Findings:

Ultrasound, CT, and MRI demonstrated a subdiaphragmatic mass with characteristic pregnancy indicators.
Laparoscopy confirmed and allowed for complete resection of the diaphragmatic ectopic pregnancy.

Implications:

This case underscores the importance of considering diaphragmatic ectopic pregnancy in women with unexplained abdominal pain.
Multimodality imaging is essential for accurate diagnosis and management of rare ectopic pregnancy presentations.
