Endoscopic Procedures I: Esophagogastroduodenoscopy

396
An Esophagogastroduodenoscopy (EGD) is a diagnostic procedure in which an endoscopist uses a flexible, lighted endoscope to visualize the upper gastrointestinal (GI) tract. The procedure includes visualizing the oropharynx, esophagus, stomach, and the first part of the small intestine, the duodenum.
During an EGD, the endoscope can be used to:

The formation of teeth, also known as odontogenesis, is a complex process that begins in utero, around the sixth week of embryonic development. There are three stages to this process: the bud stage, the cap stage, and the bell stage.
In the bud stage, the tooth germ (an aggregation of cells) starts to form in the developing jawbone. During the cap stage, the tooth germ differentiates into enamel organ, dental papilla, and dental sac, which will later develop into the tooth's enamel, dentin...

Area of Science:

  • Endodontics
  • Dental Technology
  • Regenerative Dentistry

Background:

  • Pulp canal obliteration (PCO) results from luxation injuries or other stimuli, leading to tertiary dentine formation.
  • While indicating pulp vitality, PCO can necessitate long-term root canal treatment for apical periodontitis or pulpitis.
  • Extensive PCO presents significant challenges for conventional root canal access, even for specialists.

Purpose of the Study:

  • To provide an updated review on the current status of guided endodontics (GE).
  • To explore GE as an alternative to conventional access cavity preparation for PCO.
  • To identify future research directions for improving GE techniques.

Main Methods:

  • Utilizes 3D radiological imaging (e.g., cone-beam computed tomography) and digital surface scans for virtual planning.
  • Employs static navigation (template-guided) or dynamic navigation (camera-marker system) for precise access.
  • Reviews laboratory findings and clinical case reports/series on GE accuracy and efficacy.

Main Results:

  • GE allows for virtual planning of optimal access to calcified root canal orifices.
  • Laboratory studies show promising accuracy for both static and dynamic GE navigation.
  • Clinical implementation of GE demonstrates favorable outcomes, though evidence is primarily from case reports.

Conclusions:

  • Guided endodontics is an emerging technique for managing challenging PCO cases.
  • GE shows potential for improving the accuracy and success of root canal treatments in obliterated canals.
  • Further clinical research is needed to solidify the evidence base for GE.
