Background:

  • A 73-year-old male with a history of inactive age-related macular degeneration and chronic angle-closure glaucoma (CACG) underwent femtosecond laser-assisted cataract surgery with intraocular lens (IOL) implantation and simultaneous Hydrus microstent implantation in the left eye.
  • The patient had a history of acute angle-closure glaucoma (ACG) and complicated cataract surgery in the right eye, with a failed trabeculectomy and subsequent laser peripheral iridoplasty.

Observation:

  • Two months postoperatively, the patient presented with a myopic surprise of approximately 2.0 diopters in the left eye.
  • Examination revealed differences in visual acuity and intraocular pressure (IOP) between the eyes, with a shallow anterior chamber and iris abnormalities in the left eye.

Findings:

  • The myopic surprise in the left eye, post-Hydrus microstent and cataract surgery, suggests a potential complication.
  • Findings included a fibrotic pupillary membrane, iris transillumination defects, and lens pitting in the left eye, alongside elevated IOP.

Implications:

  • This case underscores the importance of considering refractive surprises after combined glaucoma and cataract surgery.
  • Further diagnostic evaluation and tailored management strategies are crucial for optimizing visual outcomes in complex glaucoma patients.
