Area of Science:

  • Ophthalmology
  • Surgical Techniques
  • Retinal Diseases

Background:

  • Optic disc pit (ODP) can lead to serious complications like serous macular detachment and macular holes.
  • Conventional treatments for ODP-related complications, such as vitrectomy and autologous serum, may not always be effective.

Observation:

  • A patient presented with refractory serous macular detachment and a secondary full-thickness macular hole linked to an ODP.
  • Previous treatments, including pars-plana vitrectomy with internal limiting membrane peeling and autologous serum application, had failed.

Findings:

  • A novel surgical technique was employed, utilizing a silicone punctal plug to close the ODP and a human amniotic membrane graft for the macular hole.
  • Postoperatively, the serous macular detachment resolved, and the patient's visual acuity improved significantly from counting fingers to 6/38 at one year.

Implications:

  • This technique demonstrates promise as a safe and effective option for managing refractory serous macular detachment associated with ODP.
  • Further research with larger patient cohorts and extended follow-up periods is warranted to confirm the long-term safety and efficacy of this surgical approach.
