Summary

Distinguishing behavioral restitution from compensation in upper limb motor control recovery after stroke is crucial. Current kinematic and kinetic studies, while measuring movement quality, do not differentiate these recovery mechanisms.

Area of Science:

  • Neuroscience
  • Rehabilitation Medicine
  • Biomechanical Engineering

Background:

  • Differentiating behavioral restitution from compensation is key for understanding upper limb motor control recovery post-stroke.
  • Quality of Movement (QoM) measurement using kinematic and kinetic metrics during tasks may allow this disambiguation.

Purpose of the Study:

  • To identify longitudinal studies using kinematic/kinetic metrics for post-stroke reaching recovery.
  • To assess if these studies distinguish behavioral restitution from compensation.

Main Methods:

  • Systematic literature search of PubMed, Embase, Scopus, and Wiley/Cochrane Library up to July 2020.
  • Included longitudinal studies with kinematic/kinetic measurements during reaching within 6 months post-stroke.

Main Results:

  • Thirty-two longitudinal studies identified, reporting 46 kinematic metrics.
  • Most studies assessed QoM recovery but did not explicitly distinguish restitution from compensation.
  • Only one study used metrics for both performance assays and functional tasks.

Conclusions:

  • Longitudinal studies explicitly differentiating behavioral restitution and compensation post-stroke are lacking.
  • Future research should use performance assays (for restitution) and functional tasks (for restitution and compensation) to clarify these recovery patterns.
