Summary

Dedifferentiated chondrosarcoma (DDCS) with minimal or small dedifferentiated components can be aggressive, despite being difficult to detect on imaging. Thorough histologic examination is crucial for accurate diagnosis and prognosis.

Background:

  • Dedifferentiated chondrosarcoma (DDCS) is an aggressive bone malignancy.
  • DDCS typically features a large dedifferentiated (DD) component.
  • Rare cases present with minimal (<1 cm) or small (1-2 cm) DD areas.

Purpose of the Study:

  • To describe clinicopathologic features of DDCS with minimal or small DD.
  • To evaluate the prognostic significance of minimal/small DD compared to large DD (>2 cm).

Main Methods:

  • Retrospective review of 35 DDCS cases.
  • Histologic assessment of tumor components.
  • Clinical and imaging data review.

Main Results:

  • No significant difference in survival or metastasis-free survival between minimal/small DD and large DD groups.
  • Minimal/small DD were often undetected by imaging.
  • Osteosarcomatous differentiation was more common in large DD cases.

Conclusions:

  • DDCS with minimal or small DD components exhibit similar demographic and histologic features to large DD.
  • The limited extent of DD does not preclude aggressive tumor behavior.
  • Thorough gross and histologic sampling is essential for diagnosing and managing these rare DDCS variants.
