    Summary

    Sinus histiocytosis with massive lymphoadenopathy (SHML) can affect the thyroid gland, causing a painful goiter. This rare condition in the thyroid resolved on its own in a documented case.

    Background:

    • Sinus histiocytosis with massive lymphoadenopathy (SHML), also known as Rosai-Dorfman disease, is a rare benign condition.
    • While typically affecting lymph nodes, extranodal involvement can occur, including the thyroid gland.

    Observation:

    • A case of SHML presenting with a painful goiter and cervical lymphadenopathy in a white woman.
    • Biochemical hypothyroidism was noted, yet the patient remained clinically euthyroid.
    • Histological confirmation of SHML in both thyroid and cervical lymph node tissues.

    Findings:

    • SHML can manifest as a painful goiter and cervical lymphadenopathy.
    • The thyroid gland can be an extranodal site for SHML.
    • Spontaneous remission of SHML affecting the thyroid and cervical lymph nodes occurred in this patient.
