Background:

  • Many patients need optical correction after laser vision correction (LVC).
  • Conventional soft contact lenses are often inadequate for managing post-LVC irregular corneal topography or asymmetry.
  • This study explores a novel approach for visual improvement and comfort in these patients.

Observation:

  • An off-label, inverted senofilcon A (Acuvue Oasys®) lens was used in a patient experiencing discomfort with conventional soft lenses.
  • Slit lamp examination with high molecular fluorescein and a yellow filter showed improved lens centration.
  • Optical Coherence Tomography (OCT) revealed a decrease in post-lens tear film thickness from 43 microns to 35 microns.

Findings:

  • The patient achieved a visual acuity of 6/6+ in both eyes with stable vision.
  • The patient reported wearing the lenses for 9 hours daily.
  • The patient's Chronic Dry Eye Questionnaire-8 (CLDEQ-8) score improved from 22 to 15 with inverted lens wear.

Implications:

  • This case demonstrates the potential efficacy of an inverted conventional soft frequent replacement lens for post-LVC patients.
  • The technique may benefit patients with minimal asymmetric topography within the treated zone requiring refractive correction.
  • This approach offers a novel solution for improving visual acuity, comfort, and potentially managing dry eye symptoms in a specific patient population.
