Summary

Medial rectus (MR) advancement surgery effectively corrects consecutive exotropia, particularly when the MR muscle is underactive. Adjustments like bilateral MR advancement or lateral rectus (LR) recession may be needed for larger deviations.

Background:

  • Consecutive exotropia can occur after esotropia surgery.
  • Medial rectus (MR) advancement and lateral rectus (LR) recession are surgical options for strabismus.

Purpose of the Study:

  • To evaluate the clinical features and surgical outcomes of MR advancement, with or without LR recession, for consecutive exotropia.

Main Methods:

  • An interventional case series included 30 patients with consecutive exotropia (>15 PD) post-esotropia surgery.
  • Patients underwent unilateral or bilateral MR advancement with or without LR recession.
  • Ocular alignment, adduction, and exoshift were assessed at multiple follow-up points up to 6 months.

Main Results:

  • MR advancement yielded a mean dose response of 4.7 PD at 3 months and 4.55 PD at 6 months.
  • Initial success (93% at 1 week) decreased to 73% at 6 months due to postoperative exodrift.
  • Preoperative exotropia was identified as the sole contributing factor to outcomes.

Conclusions:

  • MR advancement is effective for consecutive exotropia, especially with MR underaction.
  • Bilateral MR advancement and/or LR recession are recommended for significant preoperative exodeviation.
  • Postoperative exodrift necessitates extended patient follow-up.
