    Summary

    Background:

    • Mild essential hypertension requires effective and well-tolerated treatment options.

    Purpose of the Study:

    • To evaluate the efficacy and tolerability of guanfacine monotherapy in mild essential hypertension.
    • To assess the impact of guanfacine on plasma volume, plasma aldosterone, and serum lipids.

    Main Methods:

    • Two double-blind, randomized, placebo-controlled clinical trials were conducted.
    • Patients received either 1 mg of guanfacine or placebo daily for 8 weeks.
    • Blood pressure, plasma volume, plasma aldosterone, and serum lipid profiles were monitored.

    Main Results:

    • Guanfacine significantly decreased diastolic and mean blood pressure compared to placebo (p=0.001 and p<0.0001).
    • No significant differences were observed in plasma volume or plasma aldosterone levels between groups.
    • Serum lipid profiles (cholesterol, triglycerides, lipoproteins) remained unaffected by guanfacine treatment.

    Conclusions:

    • Guanfacine monotherapy is an effective and well-tolerated initial treatment for mild essential hypertension.
    • Guanfacine does not appear to negatively impact plasma volume, aldosterone, or serum lipid levels.
    • These findings support guanfacine's role in managing mild hypertension without compromising metabolic parameters.
