Background:

Limited population-based studies exist on central auditory processing (CAP).
Perceived hearing difficulty (PHD) can occur despite normal pure-tone audiometry.
Investigating CAP in diverse populations is crucial for understanding hearing health.

Purpose of the Study:

To determine the relationship between CAP measures and PHD in an African-American population.
To assess CAP using Quick Speech-in-Noise (QuickSIN) and Dichotic Digits, Double Pairs (DDT2) tests.
To identify if CAP deficits contribute to hearing difficulties in individuals with normal audiograms.

Main Methods:

A cross-sectional study of the Jackson Heart Study (JHS) cohort.
Included African-American participants with normal hearing sensitivity (PTA-4 or PT-AF ≤ 25 dBHL).
Logistic regression analyzed the association between CAP measures (QuickSIN, DDT2) and PHD, adjusted for covariates.

Main Results:

27-28% of participants reported PHD despite normal hearing.
Increased QuickSIN scores (worse CAP) were associated with higher odds of PHD (OR 1.14-1.15).
Reduced DDT2 scores (worse CAP) were associated with higher odds of PHD (OR 0.92-0.93).

Central auditory processing deficits are significantly associated with increased odds of perceived hearing difficulty.
These findings apply to individuals with normal pure-tone hearing sensitivity within the JHS cohort.
CAP assessment may be valuable for identifying individuals experiencing hearing difficulties not detected by standard audiometry.
