Background:

Flexible diverticulotomy is a recognized treatment for Zenker's diverticulum.
This study examines the procedure's evolution since 2010 at two Austrian hospitals.

Purpose of the Study:

To evaluate the development and outcomes of flexible diverticulotomy over a decade.
To compare early and later procedural periods.

Main Methods:

Retrospective analysis of 69 flexible diverticulotomies (2010-2019).
Patients grouped into two 5-year periods for comparative statistical analysis.

Main Results:

Technical success rate of 93.5% with no lethal outcomes.
Low rates of serious (2.9%) and mild (14.5%) complications.
Dysphagia relapsed in 20.3% of patients; later period showed shorter hospital stays and longer recurrence-free intervals.

Conclusions:

Flexible diverticulotomy is safe and effective for Zenker's diverticulum.
Outcomes are linked to departmental expertise and caseload.
Procedures should be concentrated in high-volume centers.
