    Summary

    Allogeneic bone marrow transplantation offers a potential cure for multiple myeloma. This study shows promising results with complete remission in two out of three patients after intensive conditioning treatment.

    Clinical Presentation and Long-Term Survival Outcomes of Patients With Monoclonal Gammopathy of Renal Significance (MGRS): A Multicenter Retrospective Study.

    Background:

    • Multiple myeloma is a hematologic malignancy characterized by uncontrolled plasma cell proliferation.
    • Current treatments for multiple myeloma have limitations, necessitating exploration of novel therapeutic strategies.
    • Allogeneic bone marrow transplantation (BMT) is an intensive treatment modality with curative potential.

    Observation:

    • Three patients with multiple myeloma underwent allogeneic BMT.
    • Conditioning regimen included cyclophosphamide, melphalan, BCNU, and total body irradiation.
    • Post-transplant monitoring assessed for M component and monoclonal bone marrow plasma cells.

    Findings:

    • All three patients achieved complete remission, evidenced by the disappearance of serum M component and monoclonal bone marrow plasma cells.
    • One patient experienced fatal acute graft-versus-host disease (GVHD) grade IV.
    • Two patients remain in unmaintained complete remission at 4 and 20 months post-transplant.

    Implications:

    • Allogeneic BMT demonstrates significant efficacy in achieving durable remissions in multiple myeloma.
    • Careful patient selection and management of GVHD are critical for successful outcomes.
    • This approach warrants further investigation as a potentially curative option for multiple myeloma management.
