Summary

Diffusion tensor imaging (DTI) can detect microstructural changes in orbital soft tissues for thyroid-associated ophthalmopathy (TAO). DTI parameters, particularly of the medial rectus muscle, effectively indicate TAO disease severity.

Background:

  • Thyroid-associated ophthalmopathy (TAO) affects orbital soft tissues, including extraocular muscles (EOMs) and lacrimal glands (LG).
  • Accurate assessment of TAO disease severity is crucial for effective management.

Purpose of the Study:

  • To evaluate the feasibility of diffusion tensor imaging (DTI) for assessing EOM and LG changes in TAO.
  • To correlate DTI parameters with TAO disease severity.

Main Methods:

  • 74 participants (17 healthy controls, 22 mild TAO, 35 moderate-severe TAO) underwent 3-Tesla DTI.
  • Fractional anisotropy (FA) and mean diffusivity (MD) of EOMs and LG were measured.
  • Clinical ophthalmological examinations were performed, and DTI parameters were analyzed against disease severity.

Main Results:

  • TAO patients exhibited lower FA and higher MD in EOMs and LG compared to healthy controls (P < 0.05).
  • Moderate-to-severe TAO cases showed significantly reduced FA and elevated MD in these tissues.
  • Medial rectus DTI parameters differed significantly between mild and moderate-severe TAO groups (P = 0.017, P = 0.021).
  • Disease severity significantly impacted DTI parameters of orbital soft tissues.

Conclusions:

  • DTI is a valuable tool for detecting microstructural alterations in TAO-affected orbital tissues.
  • DTI findings, especially medial rectus parameters, can serve as indicators of TAO disease severity.
