Esophageal Perforation-II: Clinical Manifestations and Management

166
Esophageal perforations manifest in various clinical forms, influenced by factors such as the perforation's cause and location (cervical, intrathoracic, or intra-abdominal), the extent of contamination, and potential injury to adjacent mediastinal structures. The timing between the perforation occurrence and treatment initiation also affects the clinical presentation.

Area of Science:

  • Interventional Cardiology
  • Vascular Surgery
  • Medical Device Innovation

Background:

  • Coronary artery perforation is a serious complication during percutaneous coronary intervention (PCI).
  • Distal or small vessel perforations pose unique management challenges.
  • Current treatments include embolization with coils, fat, or microspheres.

Observation:

  • Five cases of distal coronary artery perforation were encountered.
  • These perforations occurred during PCI procedures.
  • A novel technique utilizing absorbable sutures was employed for management.

Findings:

  • The novel technique using absorbable sutures proved successful in all five cases.
  • This method effectively sealed the distal coronary artery perforations.
  • No immediate complications were reported with the suture technique.

Implications:

  • Absorbable sutures represent a promising new therapeutic option for distal coronary artery perforations.
  • This technique may offer an alternative to traditional embolization methods.
  • Further research is warranted to validate the long-term efficacy and safety of this approach.
