Summary

Trichoblastic fibroma (TF) is a rare benign tumor that can mimic sarcoma. Early surgical excision is recommended for suspicious or changing nodules to ensure accurate diagnosis and prevent potential aggressive behavior.

  • Trichoblastic fibroma (TF) is a rare benign trichogenic tumor with epithelial and mesenchymal components.
  • It exhibits partial to complete follicular induction, making its diagnosis challenging.

Observation:

  • A 35-year-old male presented with a 15-year history of a gradually enlarging lump on his left lower leg.
  • Initial investigations including ultrasound and core biopsy suggested a basaloid neoplasm, mimicking sarcoma.
  • Positron emission tomography and multidisciplinary team discussion preceded wide local excision.

Findings:

  • Histopathological examination confirmed the diagnosis of Trichoblastic Fibroma (TF).
  • The case highlights TF's potential to mimic soft tissue sarcomas.
