Cochlear implantation is feasible in patients with a persistent stapedial artery (PSA), a rare congenital anomaly. Surgical techniques safely navigated the anomaly during the procedure.

Background:

  • Persistent stapedial artery (PSA) is a rare vascular anomaly of the middle ear.
  • PSAs can present challenges during middle ear surgeries, including cochlear implantation.

Observation:

  • A case of a 25-year-old male undergoing cochlear implantation is presented.
  • Intraoperative identification of a PSA overlying the middle ear promontory.

Findings:

  • The PSA was carefully dissected and elevated from the promontory.
  • Cochlear implant electrode insertion was successfully completed via the round window niche.
  • Tragal cartilage and fibrin glue were utilized for electrode trajectory control.

Implications:

  • Cochlear implantation can be safely performed despite the presence of a PSA.
  • This case highlights successful surgical management strategies for PSA during cochlear implantation.
  • Further research may refine techniques for managing vascular anomalies in otologic surgery.
