Multiple Ruptured Aneurysms Over Basilar Artery Fenestration: Endovascular Management.

• A 62-year-old female presented with subarachnoid hemorrhage (SAH) secondary to a ruptured aneurysm on a basilar artery fenestration.
• Imaging revealed BAF with aneurysms on both limbs of the fenestration.

Findings:

• Endovascular therapy (ET) was successfully employed using coiling and flow diversion to treat the ruptured BAF aneurysm.
• Six-month follow-up angiography confirmed complete aneurysm obliteration with no neurological deficits.

Implications:

• ET offers a safe and effective treatment for complex aneurysms in challenging locations like BAF.
• This case highlights the successful management of ruptured BAF aneurysms, improving patient outcomes.
