Summary

Robotic-assisted abdominal aortic surgery (RAAS) is technically feasible for treating aortic aneurysms and occlusive disease. However, its benefits require further study through randomized trials to establish its role in vascular surgery.

Background:

  • Robotic-assisted surgery is established in many fields but not yet in vascular surgery.
  • This review focuses on robotic-assisted abdominal aortic surgery (RAAS) for aorto-iliac occlusive disease (AIOD) and abdominal aortic aneurysm (AAA).

Purpose of the Study:

  • To review and analyze the current outcomes of RAAS for AIOD and AAA.
  • To assess the feasibility and short-term results of RAAS in vascular surgery.

Main Methods:

  • A comprehensive literature search was conducted across major medical databases (Cochrane Library, OVID Medline, Embase, PubMed) from 1990 to 2020.
  • Included studies focused on RAAS for abdominal aorta and iliac arteries, excluding case reports with fewer than 5 patients.
  • Quantitative analysis focused on operative times, cross-clamping times, conversion rates, 30-day mortality/morbidity, and in-hospital stay.

Main Results:

  • Analyzed data from 850 patients across nine eligible studies.
  • For AAA: conversion rates 13.1-20%, perioperative mortality 0-1.6%, in-hospital stay 7 days.
  • For AIOD: conversion rates 0-20%, perioperative mortality 0-3.6%, in-hospital stay 5-8 days.

Conclusions:

  • RAAS is technically feasible for AIOD and AAA with acceptable short-term outcomes.
  • Benefits regarding in-hospital stay and complication rates are questionable, positioning RAAS as an "outsider procedure".
  • Randomized-controlled trials and a clear regulatory approval are essential for routine vascular surgery adoption.
