Cervical Thymic Cyst with parathyroid tissue - a diagnostic conundrum.

Background:

• Cervical thymic cysts are uncommon benign cystic lesions.
• They are frequently misdiagnosed as branchial cysts due to similar clinical presentations.
• These cysts can occur along the thymic primordia descent pathway, often with mediastinal extension.

Observation:

• A 15-year-old male presented with a 2-month history of left-sided neck swelling.
• Contrast-enhanced computed tomography (CECT) revealed a large cystic lesion with peripheral enhancement from the submandibular region to the superior mediastinum.
• Fine needle aspiration (FNA) suggested a benign inflammatory/cystic lesion, initially raising suspicion for a branchial cyst.

Findings:

• Surgical excision and subsequent histopathology confirmed a cervical thymic cyst.
• The presence of thymic tissue and Hassall's corpuscles confirmed the diagnosis.
• Associated parathyroid tissue was also identified in the specimen.

Implications:

• Accurate diagnosis of cervical thymic cysts is crucial, especially in pediatric patients presenting with neck masses.
• Awareness of this rare condition aids in differential diagnosis, alongside common entities like branchial cysts.
• Comprehensive evaluation including imaging and histopathology is essential for definitive diagnosis and appropriate management.
