Summary

Acute phlegmonous gastritis (APG) is a rare, life-threatening infection. This case report details an idiopathic APG case in a healthy individual, successfully treated with antibiotics without surgery.

Background:

Acute phlegmonous gastritis (APG) is a rare, severe gastric infection characterized by localized phlegmon formation.
APG is associated with high morbidity and mortality, often necessitating surgical intervention.
Idiopathic cases, particularly in otherwise healthy individuals, are exceptionally uncommon in medical literature.

Observation:

This case report presents an afebrile, healthy individual diagnosed with idiopathic APG.
The patient did not exhibit typical signs of systemic infection or severe illness at presentation.
Diagnostic workup confirmed the presence of phlegmonous gastritis.

Findings:

The patient's condition resolved successfully with empirical broad-spectrum antibiotic therapy.
Operative management was not required, highlighting a non-surgical treatment success.
This case underscores the potential for non-operative management in select APG cases.

Implications:

This case expands the understanding of APG's clinical spectrum and management options.
It suggests that prompt antibiotic treatment may be effective in resolving APG, even in idiopathic presentations.
Further research into risk factors and optimal non-surgical treatment protocols for APG is warranted.
