    Aggressive bone loss can occur around stable, cemented total hip replacements. This bone resorption, termed osteolysis, is linked to methylmethacrylate debris, not infection or implant loosening.

    Background:

    • Cemented total hip arthroplasty (THA) is a common procedure.
    • Implant fixation and longevity are critical for successful outcomes.
    • Osteolysis, or bone resorption, is a known complication, often associated with wear debris and implant loosening.

    Observation:

    • Four cases of extensive, localized bone resorption adjacent to rigidly anchored, cemented THA were identified.
    • No evidence of infection was found clinically, bacteriologically, or pathologically.
    • Tissue analysis revealed macrophages and giant cells invading femoral cortices.

    Findings:

    • Abundant methylmethacrylate (MMA) particulate debris was present in osteolytic tissues.
    • Polyethylene wear debris was notably absent.
