Summary

This study introduces an all-arthroscopic technique for treating patellar cartilage defects. It utilizes transpatellar retrograde reaming, avoiding open surgery and patellar eversion for improved osteochondral grafting.

Area of Science:

  • Orthopedic Surgery
  • Cartilage Repair
  • Sports Medicine

Background:

  • Patellar articular cartilage defects are challenging due to the undersurface location.
  • Current treatments often require arthrotomy and patellar eversion.
  • This limits access and necessitates a perpendicular trajectory for grafting.

Purpose of the Study:

  • To describe a novel all-arthroscopic technique for patellar osteochondral defects.
  • To present a minimally invasive approach using transpatellar retrograde reaming.
  • To facilitate effective graft fixation and recipient socket creation.

Main Methods:

  • An all-arthroscopic technique for patellar osteochondral grafting was developed.
  • Transpatellar retrograde reaming was employed for recipient socket creation.

  • This method allows for graft fixation without arthrotomy or patellar eversion.
  • Main Results:

    • The described technique enables all-arthroscopic treatment of focal patellar cartilage defects.
    • It overcomes the challenges of accessing the patellar undersurface.
    • It provides a perpendicular trajectory for osteochondral grafting.

    Conclusions:

    • This all-arthroscopic technique offers a viable solution for patellar osteochondral defects.
    • It represents a minimally invasive alternative to traditional grafting methods.
    • Transpatellar retrograde reaming facilitates precise graft placement and fixation.
