The vestibular system is a set of inner ear structures that provide a sense of balance and spatial orientation. This system is comprised of structures within the labyrinth of the inner ear, including the cochlea and two otolith organs—the utricle and saccule. The labyrinth also contains three semicircular canals—superior, posterior, and horizontal—that are oriented on different planes.

Summary

Bilateral vestibulopathy (BVP) impacts quality of life (QoL). This study found a tendency for reduced VOR gain to correlate with lower QoL, and covert saccades were linked to a less impacted QoL.

Background:

  • Bilateral vestibulopathy (BVP) is a chronic condition with variable effects on quality of life (QoL).
  • Previous research indicates a negative impact of BVP on QoL, but the precise relationship requires further investigation.

Purpose of the Study:

  • To investigate the association between vestibulo-ocular reflex (VOR) gain, saccade patterns, and patient-reported QoL in individuals with BVP.
  • To determine if specific VOR gain measurements and saccade characteristics correlate with the degree of QoL impairment.

Main Methods:

  • Ten patients diagnosed with BVP according to Bárány criteria were enrolled.
  • Vestibular function was assessed using the Head Impulse Paradigm (HIMP) and Suppression Head Impulse Paradigm (SHIMP).
  • Quality of life was measured using the Dizziness Handicap Inventory (DHI).

Main Results:

  • A trend suggested a correlation between lateral VOR gain (measured by HIMP/SHIMP) and total DHI scores.
  • The presence of covert saccades was associated with lower DHI scores, indicating a less affected QoL.
