Background:

Cardiac lymphomas are exceedingly rare, with double-hit and triple-hit lymphomas (DHLs, THLs) being particularly uncommon entities.
Studies focusing on cardiac DHL and THL are scarce, necessitating case reports to expand understanding.
Early detection and accurate diagnosis are crucial for managing cardiac malignancies.

Observation:

A 67-year-old woman presented with pulmonary embolism, leading to the incidental discovery of a right atrial mass.
The mass was surgically excised, initially presumed to be a cardiac myxoma.
Post-operative immunohistochemistry confirmed the diagnosis of triple-hit lymphoma.

Findings:

The study presents a rare case of cardiac triple-hit lymphoma.
Immunohistochemistry was pivotal in diagnosing the cardiac mass as THL.
The patient's refusal of chemotherapy resulted in a poor prognosis.

Implications:

This case underscores the importance of considering lymphoma in the differential diagnosis of cardiac masses, even with initial benign-appearing presentations.
Accurate histopathological diagnosis is critical for appropriate treatment strategies in cardiac lymphomas.
The poor prognosis associated with untreated cardiac THL emphasizes the need for timely and aggressive therapeutic interventions.
