Background:

  • The glycoprotein CD47 inhibits phagocytosis by interacting with signal regulatory protein alpha (SIRPa).
  • CD47 is implicated in immune evasion by malignant cells.
  • Understanding CD47's role in classical Hodgkin lymphoma (cHL) is critical, especially with emerging CD47-targeting therapies.

Purpose of the Study:

  • To investigate CD47 expression on Hodgkin and Reed-Sternberg (HRS) cells in cHL.
  • To correlate CD47 expression with patient prognosis, immune markers (PD-1, PD-L1, PD-L2), and SIRPa+ leukocytes.
  • To evaluate the prognostic significance of CD47 expression in cHL.

Main Methods:

  • Immunohistochemistry was used to detect CD47 and SIRPa expression on HRS cells and leukocytes.
  • Diagnostic biopsies from two independent cohorts of cHL patients (n=178) were analyzed using tissue microarrays.
  • Statistical analysis, including multivariable Cox regression, was performed to assess survival outcomes.

Main Results:

  • High CD47 expression on HRS cells significantly correlated with inferior event-free survival and overall survival in cohort I (n=136).
  • These survival findings remained significant after adjusting for known prognostic factors.
  • High CD47 expression also showed a trend towards shorter survival in cohort II (n=42) but did not reach statistical significance in multivariable analysis. No correlation was found with SIRPa+ leukocytes or PD-1/PD-L1/PD-L2 expression.

Conclusions:

  • Elevated CD47 expression on HRS cells is a significant negative prognostic factor in classical Hodgkin lymphoma.
  • CD47's role in cHL prognosis is independent of SIRPa+ leukocyte infiltration and PD-1/PD-L1/PD-L2 expression.
  • These findings support the therapeutic potential of targeting CD47 in cHL.
