Nursing Evaluation01:15

Nursing Evaluation

3.7K
The evaluation stage signals the end of the nursing process. The nurse gathers evaluative data to assess whether or not the patient has attained the expected results. Whereas the nurse collects data in the nursing assessment to identify the patient's health concerns, the evaluation stage data determines if the indicated health issues are resolved. Evaluative data collection includes two sections: the data acquired to evaluate patient outcomes and the time criteria for data collection.

Area of Science:

  • Gerontology
  • Healthcare Management
  • Clinical Nursing

Background:

  • Perley Health introduced SeeMe™: Understanding together, a program integrating frailty assessment and management with goals of care planning.
  • The program emphasizes a comprehensive approach to care for older adults facing frailty.

Purpose of the Study:

  • To evaluate the effectiveness of the SeeMe™ program in its first year of implementation.
  • To assess the impact of SeeMe™ training on healthcare providers and patient care preferences.

Main Methods:

  • A mixed-methods approach was employed for program evaluation.
  • Data collection included surveys, analysis of goals of care preferences, hospital transfer records, and interviews.

Main Results:

  • SeeMe™ training effectively educated nurses and physicians on frailty management in long-term care.
  • A 15% reduction in residents preferring hospital transfer was observed post-implementation among those with prior documented preferences.
  • No significant decrease in overall hospital transfers occurred in the first year.

Conclusions:

  • Nurses, physicians, and families reported high satisfaction with the SeeMe™ program.
  • Nurses experienced increased knowledge and confidence in managing frailty.
  • The program appears to influence residents and families towards less invasive interventions, aligning with frailty and quality of life goals.
