    Summary

    Kidney donation (nephrectomy) does not permanently harm the remaining kidney. Studies show that the kidney’s filtration capacity, or renal functional reserve, remains intact long after donation.

    Background:

    • Donor nephrectomy involves the removal of one kidney.
    • Remaining kidney compensates via hyperfiltration.
    • Long-term effects of this compensatory mechanism are not fully understood.

    Purpose of the Study:

    • To assess the long-term renal functional reserve in kidney donors.
    • To determine if compensatory hyperfiltration is damaging over time.

    Main Methods:

    • Measured glomerular filtration rates in 28 kidney donors.
    • Assessed renal function before and after an 80g protein meal.
    • Included subjects with up to 22 years post-nephrectomy.

    Main Results:

    • All subjects demonstrated a renal functional reserve.
    • No decline in renal functional reserve was observed with time post-nephrectomy.
    • Compensatory hyperfiltration appears sustained.

    Conclusions:

    • Long-term compensatory hyperfiltration after donor nephrectomy is not detrimental.
    • The remaining kidney maintains its functional reserve over extended periods.
    • Supports the safety of living kidney donation.
