Background:

  • The COVID-19 pandemic has been associated with an increased incidence of rhino-orbito-cerebral mucormycosis (ROCM).
  • India experienced a significant surge in ROCM cases during the second wave of the COVID-19 pandemic.

Purpose of the Study:

  • To describe the prevalence of ROCM in SARS-CoV-2 infected patients during India's second COVID-19 wave.
  • To detail diagnostic procedures and discuss challenges in managing ROCM from diagnosis to treatment decisions.

Main Methods:

  • A retrospective multicentre case series study was conducted across six hospitals in India.
  • Data were collected from 1546 patients admitted with SARS-CoV-2 infection.

Main Results:

  • ROCM was confirmed in 38 (2.5%) of the patients.
  • The average diagnostic delay was 16 days. 50% of ROCM patients had type 2 diabetes, often treated with hypoglycemic agents.
  • Median blood glucose levels were high (320 mg/dl). Mortality was 5%, with 16% experiencing irreparable functional loss.

Conclusions:

  • Poorly controlled blood glucose, unmanaged steroid administration, and excessive antibiotic use may create conditions favorable for ROCM.
  • These factors, alongside SARS-CoV-2 infection management, likely contributed to the observed increase in ROCM.
