Summary
Bladder osteosarcoma is rare and has a poor prognosis. Early detection and transurethral resection of bladder tumor alone led to long-term survival without recurrence in one patient.

- Due to the patient's advanced age and comorbidities, radical cystectomy was deemed unsuitable.
- The patient underwent a second transurethral resection of bladder tumor, achieving complete tumor removal.
- At 29 months post-surgery, the patient remains alive with no evidence of recurrence.
