Summary

Photodynamic therapy (PDT) offers a new approach for treating peripheral lung cancer. This minimally invasive technique successfully eradicated early-stage non-small cell lung cancer in a patient unsuitable for surgery.

Background:

  • Increasing lung cancer screening identifies more early-stage peripheral tumors, posing treatment challenges for patients ineligible for surgery or radiation.
  • Emerging bronchoscopy techniques offer potential for treating peripheral lung tumors with minimally invasive methods.

Observation:

  • A case study involved a patient with primary non-small cell lung cancer located in the lung periphery.
  • Magnetic navigational bronchoscopy was used to precisely place a diffusing fiber for interstitial PDT.
  • The treatment involved a single illumination session after intravenous Photofrin® administration, with no immediate complications.

Findings:

  • Pathology after a planned lobectomy confirmed complete tumor necrosis and absence of viable cancer cells 30 days post-PDT.
  • The patient showed no evidence of disease at 90-day follow-up, indicating successful treatment.
  • Interstitial PDT, guided by magnetic navigation, proved effective for a solitary peripheral lung tumor.

Implications:

  • This case demonstrates the potential of interstitial PDT as a viable, well-tolerated outpatient therapy for peripheral lung cancers.
  • PDT may expand treatment options for patients with early-stage lung cancer who are not candidates for conventional therapies.
  • Further research into navigational bronchoscopy-guided PDT could enhance its application for treating difficult-to-reach lung tumors.
