The enhancement index of symptomatic plaques is a key imaging marker for predicting early-onset post-stroke depression (PSD). This finding aids in the early diagnosis of PSD in stroke patients.
Area of Science:
- Neurology
- Radiology
- Psychiatry
Background:
- The relationship between imaging characteristics of intracranial atherosclerotic plaques and early-onset post-stroke depression (PSD) remains unclear.
- Investigating imaging biomarkers for PSD is crucial for timely intervention.
Purpose of the Study:
- To determine if imaging features of symptomatic intracranial atherosclerotic plaques predict early-onset PSD.
- To identify independent predictors of early-onset PSD in ischemic stroke patients.
Main Methods:
- 76 ischemic stroke patients underwent high-resolution vessel wall MRI (HR-VWI).
- Patients were classified into PSD and non-PSD groups based on DSM-V criteria and HAMD-17 scores at 14 days post-stroke.
- Analysis included comparison of clinical data, plaque imaging features (enhancement index, remodeling index, plaque surface irregularity), and logistic regression to identify predictors.
Main Results:
- 36 patients had early-onset PSD.
- Symptomatic plaque enhancement index (EI) and infarct volume were significantly higher in the PSD group (P < 0.05).
- Symptomatic plaque EI was an independent predictor of early-onset PSD (OR = 1.034, P = 0.001), even after adjusting for infarct volume. EI, infarct volume, and HAMD-17 scores were positively correlated.
Conclusions:
- The enhancement index of symptomatic plaques is a significant independent imaging biomarker for early-onset PSD.
- This imaging marker can assist in the early diagnosis of post-stroke depression.
