Implementing targeted patient safety strategies significantly increased reporting rates in a Japanese hospital. A strong safety culture and adequate strategy implementation are key to successful patient safety initiatives.
Area of Science:
- Healthcare Management
- Patient Safety
- Implementation Science
Background:
- Healthcare systems are complex, leading to adverse events.
- Patient safety reporting systems are crucial but face cultural implementation barriers.
- Implementation science offers tools to analyze and overcome these barriers.
Purpose of the Study:
- To determine the effect of implemented strategies on patient safety report numbers.
- To identify determinants of successful patient safety reporting system implementation.
- To analyze strategy effectiveness using an implementation framework.
Main Methods:
- Mixed-methods analysis at Fujita Health University Hospital (FHUH), Japan.
- Utilized the Consolidated Framework for Implementation Research (CFIR) to identify strategies from internal documents.
- Analyzed electronic reporting system data (2004-2020) alongside staff and hospital volume data.
Main Results:
- Over 110,000 reports analyzed; significant increases observed after concentrated strategy implementation periods (2007, 2016).
- Identified 74 contributing strategies across CFIR domains: Intervention Characteristics (12), Outer Settings (20), Inner Settings (21), Individual Characteristics (8), and Process (13).
- Reporting rates showed stagnation between 2009-2012, highlighting the need for sustained strategic efforts.
Conclusions:
- A robust safety culture was fostered at FHUH during the study period.
- A direct relationship exists between the number of implemented strategies and the development of a reporting culture.
- The study underscores the necessity of adequate and intensive strategies for successful patient safety reporting system implementation.
