When You Cannot Go With the Flow: A Case Report of May-Thurner Syndrome

May-Thurner syndrome (MTS) can cause left leg swelling and pain due to iliac vein compression. Further CT imaging is recommended if initial ultrasounds for deep vein thrombosis are negative.

Background:
- May-Thurner syndrome (MTS) involves extrinsic compression of the left iliac vein, often leading to deep vein thrombosis (DVT).
- MTS predominantly affects women aged 20-50, presenting with unilateral leg swelling and pain.
Observation:
- A patient presented with back and left lower extremity pain and swelling.
- Initial duplex ultrasound was negative for deep venous thrombus.
Findings:
- Computed tomography (CT) revealed a venous thrombus in the left common, internal, and external iliac veins.
- Diagnosis of MTS was confirmed despite a negative initial ultrasound.
Implications:
- CT venography should be considered for unilateral lower extremity swelling when initial ultrasounds are negative for DVT.
- Early diagnosis and appropriate imaging are crucial for managing May-Thurner syndrome.
