Background:

The upper brainstem tegmentum's complexity hinders functional localization.
The precise role and laterality of subregions controlling consciousness and urinary continence are not fully understood.

Observation:

A patient presented with a right pontine tegmental syndrome due to intraparenchymal hemorrhage.
Hemorrhage extended into the fourth ventricle and affected the caudal midbrain and right pontine tegmentum.
The patient maintained consciousness but developed complete urinary retention with over 1,000 mL residual bladder volume.

Findings:

Right pontine tegmentum lesions can cause significant urinary retention.
Consciousness was preserved despite extensive brainstem involvement.

Implications:

This case suggests the right pontine tegmentum is critical for the micturition reflex pathway.
Understanding brainstem functional anatomy is crucial for diagnosing and treating neurological disorders affecting bladder control.
