    Summary

Infective endocarditis is a severe complication for patients with hypertrophic obstructive cardiomyopathy (HOCM), often leading to fatal septic shock. Early surgical intervention should be considered for acute cases.

    Area of Science:

    • Cardiology
    • Infectious Diseases
    • Pathology

    Background:

    • Hypertrophic obstructive cardiomyopathy (HOCM) presents unique challenges in managing valvular complications.
    • Infective endocarditis is a serious condition affecting heart valves, particularly in patients with pre-existing cardiac abnormalities.

    Observation:

    • A case report details a 37-year-old female with HOCM who developed infective endocarditis of the aortic and mitral valves.
    • The patient succumbed to septic shock, highlighting the critical nature of this combined condition.

    Findings:

    • Infective endocarditis causing acute aortic and/or mitral valve insufficiency is a severe complication in HOCM patients.
    • The compromised left ventricle in HOCM cannot adequately compensate for acute volume overload, increasing mortality risk.
    • Standard treatments like nitrates for heart failure and catecholamines for septic shock pose significant challenges in this patient group.

    Implications:

    • Preventive strategies for infective endocarditis are crucial for HOCM patients, similar to other valvular heart diseases.
    • Early surgical intervention must be a key consideration for patients presenting with acute endocarditis and HOCM.
    • This case underscores the need for tailored management strategies for complex cardiovascular conditions.
